- On paper, in the office. At registration or check-in, patients complete and sign our patient communication consent form, which includes a section authorising appointment reminder text messages to the mobile number they provide. Signed forms are retained in the patient record.
- Online, using the form below. Patients enter their own mobile number and tick the consent box, which is not pre-checked.
Consent to receive text messages is not a condition of treatment or of purchasing any goods or services.
